Required Group Visit Forms
We collect these forms and information before your arrival to help our Programming and Kitchen staff prepare for your visit.
Planning Your Visit (due 30 days before arrival)
Finalizing the Trip (due 14 days before arrival)
- Final Numbers & Roster
These numbers are used to finalize your schedule and for billing - Certificate of Insurance
Please review your agreement addendum for required coverages
Required Participant Forms
You are required to provide these completed forms upon arrival for all attendees (Youth and Adult)
UW Assumption of Risk
Photo Release
In this form, there is an option to say yes or no to having photos taken or used by Upham Woods, but this must be completed to ensure we have the individual’s or the youth’s parents’ decision.
Optional Forms
Tell us about your group. We are required to ask for your group’s demographics to support our reporting.
